The program emphasizes trauma-informed, family-centered approaches that support both infant well-being and caregiver capacity. Nursing staff collaborate with hospital teams, primary care providers, and specialty clinicians to ensure continuity of care, reinforce discharge plans, and connect families to appropriate medical, behavioral health, and community-based resources.
By addressing clinical, emotional, and logistical needs during this high-risk transition period, the NICU-to-home transition support program aims to improve patient safety, reduce avoidable readmissions, and strengthen long-term outcomes for infants and families.

We recognize that medical needs don’t exist in isolation. Families, caregivers, and individuals navigating complex health situations often experience anxiety, stress, trauma, and emotional exhaustion.
Mitten Touch incorporates trauma-informed mental health support into our care — including screening, education, emotional support, and coordination with behavioral health providers when needed.

Mitten Touch provides nurse-led support to individuals transitioning through vulnerable periods of care, including those living in community-based housing programs such as recovery residences and halfway houses.
Services focus on health stabilization, mental health support, care coordination, medication education, and connection to community and clinical resources — helping individuals remain safe, supported, and engaged in their recovery.
